La douleur chez le nouveau-né: conséquences à long terme et prise en charge

Résumé
La douleur est une expérience quotidienne pour les nouveau-nés hospitalisés dans les services de néonatologie, et bien que sa prise en charge soit un souci permanent pour les équipes, elle est encore bien insuffisante. Or, on sait désormais que la douleur en période néonatale a des conséquences à long terme. Comme pour l’adulte et l’enfant plus âgé, un traitement adéquat passe par une reconnaissance et une évaluation adaptées à l’âge et à la pathologie. La plupart des médicaments utilisés chez le nouveau-né sont ceux utilisés chez l’enfant plus grand, bien que nombre d’entre eux n’aient pas fait l’objet d’étude spécifique. Seuls les moyens non pharmacologiques, en particulier les solutions sucrées, ont une spécificité néonatale et des recommandations spécifiques.

Abstract
Pain is a daily experience for the neonate in the neonatal intensive care, and despite a permanent concern for caregivers, its treatment is still inadequate. Evidence is increasing for long-term adverse effects and consequences of pain in early life. As for adults and children, the first step of an effective treatment is a good recognition and evaluation of pain. Few treatments in the neonates have been specifically studied for this population, and except the non-pharmacological treatment, all of them are extrapolation of those used in older population.
