Zusammenfassung
Wir sahen eine 72-jährige nierentransplantatierte Patientin (CKD-Stadium G3b A2T) mit akut-auf-chronischem Nierenversagen unter oraler Aciclovirtherapie. Risikofaktoren für die Kristallnephropathie waren eingeschränkte Nierenfunktion, Hypovolämie sowie fehlende Dosisanpassung. Unter Infusionstherapie verbesserte sich die Nierenfunktion 11 Tage nach Beendigung der Aciclovirtherapie. Dieser Fall demonstriert, dass es bei entsprechenden Risikofaktoren bereits unter oraler Aciclovirtherapie zur Nierenschädigung kommen kann und dass auch bei normalem Sediment an eine Aciclovirnephropathie gedacht werden sollte.
Abstract
A 72-year-old kidney transplant recipient (CKD G3b A2T) presented with acute-on-chronic renal failure following oral administration of acyclovir. Risk factors were chronic renal insufficiency, hypovolemia and inappropriately dosed acyclovir therapy. Withdrawal of cyclovir treatment in combination with oral and parenteral hydration resulted in an improvement of renal function. This case report demonstrates that oral acyclovir and not only intravenous administration causes renal failure especially with underlying comorbidities and inappropriate dosage. Acyclovir nephropathy must be considered even if urinalysis does not show crystals.
