Introduction

Hyperglycaemia is associated with increased mortality in critical illness, and is related to excessive gluconeogenesis, relative insulin deficiency, and insulin resistance. Intensive insulin therapy (IIT), aiming for tight glycaemic control (TGC) [blood glucose (BG) 4.4–6.1 mmol/l], has resulted in a significant mortality and morbidity reduction compared to those treated with conventional treatment [1, 2].
