Zusammenfassung
In der klassischen Definition der Kollagenosen werden fünf rheumatische Krankheitsbilder mit Beteiligung von Bindegewebe zusammengefasst. Hierzu zählen der systemische Lupus erythematodes (SLE), die Polymyositis (PM) bzw. Dermatomyositis (DM), das Sjögren-Syndrom (Sj), die Sklerodermie (SSc) und das Sharp-Syndrom („mixed connective tissue disease“, MCTD). Daneben kann bei einer großen Anzahl von Patienten das Krankheitsbild nicht eindeutig zugeordnet werden oder es treten im Zeitverlauf Symptome anderer Kollagenosen auf. Bei diesen Patienten spricht man von einem Overlap-Syndrom. Diese Vielzahl an Erkrankungen können zudem recht unterschiedliche Pathologien des Atmungsapparats auslösen. Hiervon können die Atemmuskulatur, die Pleura, das Lungenparenchym in Form von Pneumonitis oder Lungenfibrose und schließlich das Gefäßsystem betroffen sein. Dieser Beitrag gibt einen Überblick zu diesen Manifestationsformen und zur Therapie der kollagenoseassoziierten interstitiellen Lungenerkrankungen (ICD).
Abstract
The classical definition of collagenosis encompasses five rheumatic disease symptomatics with involvement of connective tissue. These include systemic lupus erythematosus (SLE), polymyositis (PM) or dermatomyositis (DM), Sjögren’s syndrome (Sj), scleroderma or systemic sclerosis (SSc) and Sharp’s syndrome or mixed connective tissue disease (MCTD). Additionally, in a large number of patients the symptoms cannot be clearly assigned or symptoms of other forms of collagenosis occur in the course of time. For these patients this is known as an overlap syndrome. This multitude of diseases can also trigger completely different pathologies of the respiratory apparatus so that the breathing musculature, the pleura, the lung parenchyma in the form of pneumonitis or pulmonary fibrosis and ultimately the vascular system can all be affected. This article gives an overview of these manifestation forms and on the therapy of collagenosis-associated interstitial lung diseases (ILD).
