Zusammenfassung
Opioide sind in der Behandlung starker akuter und chronischer Schmerzen ein wichtiger Bestandteil der medikamentösen Schmerztherapie. Sie unterscheiden sich in ihrer analgetischen Wirksamkeit, ihrem Nebenwirkungsprofil und dem Risiko für Wechselwirkungen. In diesem Beitrag werden die pharmakokinetischen Interaktionen der einzelnen Substanzen auf Ebene der Biotransformation besprochen und welche klinischen Konsequenzen sich hieraus ergeben können. Für die Einschätzung des Interaktionspotenzials der Opioidanalgetika auf Ebene der Cytochrom-P450(CYP)-Enzyme ist das Verhalten der Wirkstoffe zu den beiden Isoenzymen CYP2D6 und CYP3A4 von maßgeblicher Bedeutung.
Abstract
Opioids are an important component of the drug treatment of patients with acute and chronic pain. They differ in effectiveness, side effect profile and the risk of interactions. In this article the pharmacokinetic mechanisms of drug-drug interactions at the level of biotransformation are described and the clinical consequences which can arise are discussed. The relation of the active components to the two isoenzymes CYP2D6 and CYP3A4 is of major importance for assessing the potential drug-drug interactions of opioid analgesics at the level of the cytochrome P450 enzyme.

Fallbeispiel 1
Ein Neugeborenes nach unauffälliger Schwangerschaft und Termingeburt wird ab dem 7. Lebenstag zunehmend lethargisch. Am 13. Lebenstag trifft das Notfallteam auf ein zyanotisches, lebloses Baby. Jegliche Wiederbelebungsversuche sind erfolglos. Die Mutter des Kindes erhielt nach der Geburt als Schmerztherapie Codein 60 mg alle 12 h bis zum 12. postpartalen Tag. Eine nachträgliche Phänotypisierung von CYP2D6 der Mutter ergibt, dass sie ein ultraschneller Metabolisierer ist. Es ist zu vermuten, dass im Blut und in der Muttermilch rasch sehr hohe Konzentrationen Morphin aus Codein gebildet wurden und es zu der Vergiftung des gestillten Säuglings gekommen ist.
Fallbeispiel 2
Einem älteren Patienten unter Fentanylpflastertherapie wird vom Dermatologen zur Behandlung einer ausgeprägten Pityriasis versicolor eine systemische Therapie mit dem Antimykotikum Itraconazol verordnet. Nach zwei Tagen wird der Patient bewusstlos mit einer schweren Atemdepression aufgefunden. Nach Applikation von Naloxon erlangt er sofort wieder das Bewusstsein. Itraconazol als starker CYP3A4-Inhibitor hemmt den Metabolismus des CYP3A4-Substrats Fentanyl. Ohne Reduktion der Pflasterstärke ist es zu einer Überdosierung des Opioidanalgetikums gekommen.
