Zusammenfassung.
Persönlichkeitsorganisation und Therapieprozess bei polyvalent abhängigen Patienten werden anhand einer vergleichenden Analyse von 15 Einzelverläufen untersucht. Dabei wird Dissozialität als vorherrschender klinischer Zug herausgearbeitet und genetisch mit einer genau umschriebenen traumatischen Erfahrung sowie einer daraus resultierenden Externalisierung von Über-Ichanteilen zusammengebracht. Im Therapieprozess entfaltet die Dissozialität zunächst eine destruktive Dynamik, die zu einem Scheitern der therapeutischen Beziehung führt. Kann die Therapie danach jedoch wiederaufgegriffen werden, so ist es den Patienten jetzt möglich, sich auf einen Therapieprozess einzulassen, bei dem es um neue Identifizierungen und die Integration von Über-Ichanteilen geht. Gegen Ende der Therapie besteht die Gefahr, dass der Patient den Therapieerfolg wieder zerstört, wenn nicht auf besondere Schwierigkeiten beim Trennungsprozess geachtet wird.
The therapy process of drug-addicted patients in an inpatient setting. The impact of antisocial tendencies on the therapeutical relation
Summary.
The personality organisation and the process of therapy of drug-addicted patients are examined by the comparative analysis of 15 single cases. The antisocial tendency is emphasised as the main clinical feature. It is related to specific genetic traumata and the externalisation of parts of the superego. During the therapeutical process, the antisocial tendency destroys the therapeutical relation. If it is possible to continue therapy beyond this point, the patients can now enter into a process, in which new identifications and the integration of parts of the superego are involved. Approaching the end of the therapy, patients, again, tend to destroy the results of therapy if one does not consider specific problems of the separation process.
