Zusammenfassung

Ethylenglykol ist eine per se nicht toxische Substanz, die im menschlichen Organismus in hoch-toxische Metabolite umgewandelt wird. Die akute Intoxikation ist durch eine schwere metabolische Azidose, Vigilanzstörung, respiratorische Insuffizienz und Nierenversagen gekennzeichnet. Bereits die Aufnahme einer geringen Menge Ethylenglykol kann unbehandelt zum Tod führen. Frühzeitige Diagnosestellung und Therapiebeginn können deshalb lebensrettend sein. Wir berichten über einen 47-jährigen Patienten, der seine Ethylenglykolvergiftung dank frühzeitiger intensivmedizinischer Behandlung überlebte.

Abstract

Ethylene glycol is primarily a nontoxic substance which is metabolized into highly toxic products. The acute intoxication is characterized by marked metabolic acidosis with increased anion gap, reduced levels of consciousness, respiratory insufficiency and renal failure. Even small accounts of ingested ethylene glycol can lead to a fatal outcome if not adequately treated. Thus, early diagnosis and treatment are life saving. We report a 47-year-old male patient who survived a severe ethylene glycol intoxication with early onset and broadly targeted intensive care treatment.
