Intravitreale Chemotherapie bei okulozerebralem Lymphom

Zusammenfassung

Das okulozerebrale Non-Hodgkin-Lymphom (NHL) wird heute meist primär mit systemischer Chemotherapie behandelt.Nach initial guter Tumor-Regression sind Rezidive aber häufig.Zur Behandlung von Rezidiven kann eine Strahlentherapie eingesetzt werden. Nach primär gutem Ansprechen ist aber auch nach Radiatio häufig erneutes Tumorwachstum zu beobachten.Wegen radiogener Komplikationen an Zerebrum und Auge kann die Radiatio nur einmalig eingesetzt werden.Bei okulärem Rezidiv eines okulozerebralen NHL kann eine intravitreale Chemotherapie (Methotrexat 0,4 mg und Dexamethason 0,4 mg, einmal wöchentlich für 4 Wochen,danach einmal monatlich als Erhaltungstherapie) durchgeführt werden.Innerhalb weniger Wochen kann nach dieser Therapie ein Einschmelzen der Tumormassen mit evtl.erheblicher Visusbesserung erreicht werden.Bis fast 2 Jahre nach Beginn der intravitrealen Injektionen wurde Rezidivfreiheit beschrieben. Die intravitreale Chemotherapie stellt eine wirksame Behandlungsstrategie für okuläre Rezidive des okulozerebralen NHL nach Chemotherapie und Radiatio dar.

Abstract

Primary CNS and intraocular non-Hodgkin's lymphoma is currently mostly treated with systemic chemotherapy.After initially successful tumor regression, recurrence is common and usually treated with radiotherapy. However, after good primary therapeutic response, new tumor growth is frequently observed. Because of actinic complications, radiotherapy can be applied only once.For an intraocular recurrence of primary CNS and intraocular lymphoma, intravitreal chemotherapy (0.4 mg methotrexate and 0.4 mg dexamethasone weekly for 4 weeks and once a month thereafter) can be performed. A few weeks after initiation, regression of intraocular tumors and eventually improvement of vision can be achieved.Eyes remained free of recurrence up to 2 years after initiation of intravitreal injections. Intravitreal chemotherapy is an effective treatment for ocular recurrence of primary CNS and intraocular lymphoma after systemic chemotherapy and radiotherapy.
