Progredienter Schulterschmerz eines 35-jährigen Hobbysportlers

Zusammenfassung

Differenzialdiagnostisch stehen bei Schulterschmerzen eines jüngeren Patienten die glenohumerale Instabilität, eine Pathologie des Bizepssehnenankers (SLAP) oder eine Rotatorenmanschettenläsion im Vordergrund. Im vorgestellten Fall waren die Beschwerden des Amateursportlers Ausdruck eines ausgedehnten Knorpelschadens sowohl am Humeruskopf als auch korrespondierend am Glenoid. Der arthroskopische Befund zeigte zudem multiple freie Gelenkkörper, die auf eine traumatische Genese des Knorpeldefekts hindeuteten.

In einer zweizeitigen Intervention mit glenoidaler Mikrofrakturierung und humeralseitiger matrixassoziierter Chondrozytentransplantation (mACT) wurde versucht, ein Voranschreiten des bifokalen osteochondralen Defekts nicht nur zu bremsen, sondern orthobiologisch aufzubauen.

Abstract

Common causes of shoulder pain in younger patients are glenohumeral instability, instability of the long biceps anchor (SLAP) or rotator cuff lesions. In the present case of an amateur throwing athlete a bifocal, glenohumeral osteochondral lesion produced progressive shoulder pain. Multiple arthroliths found during arthroscopy may be signs of a traumatic origin. The osteochondral defects were addressed by a two-stage intervention including microfracture of the glenoid and matrix-associated autologous chondrocyte transplantation of the humeral head.
