Zusammenfassung

Vorgestellt wird ein 9-jähriges Mädchen mit rezidivierenden Kopfschmerzen, Gewichtsabnahme und arterieller Hypertonie aufgrund einer hyperthyreotischen Stoffwechsellage. Die Befunde deuteten auf einen M. Basedow hin, die TRAK (TSH-Rezeptor-Antikörper, TSH: Thyreoidea stimulierendes Hormon) waren aber zunächst negativ und ließen sich erst in Verdünnung nachweisen, denn durch den High-Dose-Hook-Effekt (Hook-Effekt) kam es bei hoher Konzentration des zu untersuchenden Agens zu einem falsch-negativen Wert.

Abstract

The case of a 9-year-old girl with relapsing cephalgia, weight loss and arterial hypertension due to hyperthyroidism is presented. Clinical findings suggested Graves’ disease, but TRAbs (TSH receptor antibodies) were negative at first. They were only detected after the serum had been diluted because the high concentration of TRAbs in the serum led to a false-negative result due to the high-dose hook effect.
