Zusammenfassung
Zuverlässige epidemiologische Daten zeigen eine insgesamt hohe Frequenz von primären idiopathischen Kopfschmerzen im Kindesalter. Sie sind damit ein wichtiges Thema der Pädiatrie und Neuropädiatrie. Die Migräne stellt eigene Anforderung an die Diagnose und eine erfolgreiche Therapie. Für ihre Akuttherapie im Kindesalter zeigen neben den bekannten Medikamenten Paracetamol und Ibuprofen nasale Triptane (Serotoninrezeptoragonisten) eine studienbelegte Effizienz. In der medikamentösen Migräneprophylaxe bei Kindern (sehr selten notwendig) liegen die besten evidenzbasierten Daten für Flunarizin vor. Die positiven Ergebnisse der Antiepileptika Valproinsäure und Topiramat in der medikamentösen Prophylaxe bei Erwachsenen werden zurzeit aktiv diskutiert, eindeutige klinische Erfahrungen im Kindesalter fehlen. Insgesamt bedarf es für das Kindesalter weiterer evidenzbasierter Studien, um den Einsatz von bislang nur im Erwachsenenalter validierten Medikamenten zu begründen. Für die Migräne im Kindesalter ist die schnelle und ausreichende Schmerzmittelgabe – eingebettet in ein multimodales Therapiekonzept – Grundlage der Behandlung.
Abstract
Epidemiologic data from recent years demonstrate the high frequency of headache, including migraine, in children. Headaches in children are a leading concern for pediatricians and neuropediatricians. Migraine offers special concerns for diagnosis and therapy. In addition to the well-known medications paracetamol and ibuprofen, the nasal sumatriptans have demonstrated good efficacy for acute treatment of migraine headache in children. For preventive treatment in children—which is rarely required—flunarizin has shown the best evidence-based results. Limited data are available on significant clinical effects of the anticonvulsants valproic acid and topiramate for preventive treatment in children, although these are discussed actively for adults. Therefore, further evidenced-based clinical studies of children with migraine are needed to validate new medications. Treatment for children with migraine should be based on fast and sufficient medication in a multimodal therapeutic concept.
