Zusammenfassung
Die seltenen Papillentumoren entwickeln sich aus den Strukturen der Papilla Vateri und präsentieren sich dementsprechend als heterogene Tumorgruppe. Bei den gutartigen Tumoren ist das intestinal differenzierte Adenom das häufigste, bei den bösartigen das intestinal differenzierte Papillenkarzinom. Karzinome mit histologisch pankreatikobiliärer Differenzierung haben eine schlechtere Prognose. Selten finden sich neuroendokrine und mesenchymale Papillentumoren. Die Diagnostik erfolgt durch Seitblickendoskopie und Biopsie. Bei Malignomverdacht ist ein komplettes Staging mittels Computertomographie und Endosonographie zur Feststellung der lokalen Tumorausdehnung angezeigt.
Adenome werden endoskopisch mittels Schlingenpapillektomie abgetragen, bei Papillenkarzinomen ist die Pankreaskopfresektion mit systematischer Lymphadenektomie indiziert. Patienten mit Papillenkarzinomen profitieren wahrscheinlich von einer adjuvanten Therapie, diese sollte im interdisziplinären Konsens und unter Berücksichtigung der histologischen Differenzierung festgelegt werden.
Abstract
Papillary tumors originate from the various structures of the ampulla of Vater; therefore, these rare tumors represent a heterogeneous group of tumor entities. Intestinal differentiated adenomas are the most common benign lesions, whereas intestinal differentiated papillary carcinomas are the most common malignant tumors. Carcinomas with pancreaticobiliary differentiation have a poorer prognosis. Mesenchymal and neuroendocrine tumors are among the least frequent papillary tumors. Diagnosis is performed by side-view upper endoscopy and biopsy. In cases of suspected malignancy a complete staging with computed tomography (CT) and endoscopic ultrasound scanning is indicated to determine local tumor spread.
Adenomas are removed by endoscopic snare papillectomy whereas the therapy of choice for papillary carcinomas is pancreatic head resection with systematic lymphadenectomy. Patients with papillary carcinomas are most likely to benefit from adjuvant therapy, which should be determined in an interdisciplinary consensus conference considering the histological differentiation of the tumor.

Papillenkarzinome verhalten sich wie Kolonkarzinome und sollten entsprechend behandelt werden.
Papillenkarzinome sind wie Pankreaskarzinome stets mit Gemcitabin zu behandeln.
Für eine adjuvante Chemotherapie liegt eine klare Evidenz vor.
Der Nutzen einer Chemotherapie überwiegt nachgewiesenermaßen die zu erwartende Morbidität.

Wie unterscheidet sich das Papillenkarzinom vom Pankreaskarzinom in Bezug auf die Therapie und Prognose?
Das Papillenkarzinom wird oft früher entdeckt, da es aufgrund der Lokalisation obstruktive Symptome verursachen kann.
Es handelt sich beim Papillenkarzinom um einen semimalignen Tumor.
Für das Papillenkarzinom stehen deutlich mehr multimodale Therapieformen wie z. B. Chemotherapien zur Verfügung.
Das Papillenkarzinom ist im Gegensatz zum Pankreaskarzinom besonders strahlensensibel.
Das Papillenkarzinom metastasiert sehr selten und ist somit regelhaft durch eine Operation behandelbar.
Eine Patientin klagt seit mehreren Wochen über abdominelle Beschwerden. In der daraufhin durchgeführten Endoskopie zeigt sich der Verdacht auf ein gut zugängliches Papillenadenom. Welcher Schritt sollte nun erfolgen?
