Hintergrund
Die Einschätzung, dass E‑Zigaretten weniger schädlich sind als herkömmliche Tabakerzeugnisse, kann den Konsum von E‑Zigaretten beeinflussen.
Ziel der Arbeit
Es wurden drei Fragestellungen untersucht: (1) Wie werden die Gesundheitsgefahren durch E‑Zigaretten in verschiedenen Bevölkerungsgruppen eingeschätzt? (2) Erklären soziodemografische Variablen Unterschiede in der Gefahreneinschätzung von E‑Zigaretten? (3) Sagt die Einschätzung der Gesundheitsgefahren die Nutzung von E‑Zigaretten als Mittel zur Tabakentwöhnung voraus?
Methoden
Als Datenbasis diente der Epidemiologische Suchtsurvey (ESA) 2015 mit einer Stichprobe von n = 9204 18- bis 64-Jährigen (Antwortrate 52,2 %). Die Datenerhebung erfolgte mittels eines Methodenmixes (schriftlich, telefonisch, webbasiert). Die Einschätzung der Gesundheitsgefahren von E‑Zigaretten wurde im Vergleich zu konventionellen Zigaretten erfasst (schädlicher, genauso schädlich, weniger schädlich, weiß nicht). Zur Datenanalyse wurden logistische Regressionen durchgeführt.
Ergebnisse
E-Zigaretten wurden von Frauen und älteren Personen als genauso schädlich, von Personen mit einer niedrigeren Bildung als schädlicher und von Rauchern als schädlicher oder genauso schädlich wie Tabakzigaretten eingeschätzt. Die Wahrscheinlichkeit, E‑Zigaretten als Hilfsmittel zur Tabakentwöhnung zu nutzen, war höher, wenn Individuen diese als weniger schädlich als klassische Tabakprodukte einschätzten.
Diskussion
Nur einem Drittel der Bevölkerung ist bekannt, dass E‑Zigaretten nach der aktuellen Forschung weniger gesundheitsschädlich sind als herkömmliche Zigaretten. Die subjektive Einschätzung der Gesundheitsgefahren ist mit der Nutzung von E‑Zigaretten zur Tabakentwöhnung assoziiert.
Abstract
Background
The perception that e‑cigarettes are less harmful than traditional tobacco products can influence the consumption of e‑cigarettes.
Objectives
Three questions were examined: (1) How do different population groups perceive health risks of e‑cigarettes? (2) Do sociodemographic variables explain differences in the risk assessment of e‑cigarettes? (3) Does the perception of health risks predict the use of e‑cigarettes for smoking cessation?
Methods
Data came from the 2015 Epidemiological Survey of Substance Abuse (ESA) with a sample size of n = 9204 participants, aged 18 to 64 years (response rate 52.2%). Data were collected by telephone, online, or by written questionnaires. Assessments of risk perception of e‑cigarettes and conventional cigarettes (more harmful, just as harmful, less harmful, do not know) were compared. Descriptive statistics and logistic regressions were performed.
Results
Individuals with lower education rated e‑cigarettes as more harmful. Older people and women perceived e‑cigarettes as just as harmful. Smokers considered e‑cigarettes to be more harmful than or just as harmful as conventional tobacco products. The likelihood of using e‑cigarettes for smoking cessation was higher if people thought they were less harmful than conventional cigarettes.
Conclusions
Only one-third of the population knows that e‑cigarettes are less harmful to health than conventional cigarettes. The perception of health risks is related to the usage of e‑cigarettes for smoking cessation.

Der Epidemiologische Suchtsurvey (ESA) wird seit 1980 in regelmäßigen Zeitabständen erhoben. Der ESA 2009 wurde vom Ethikkomitee der Deutschen Gesellschaft für Psychologie bewilligt (DGPs; Reg.-No: GBLK06102008DGPS). Da der ESA 2009 und der ESA 2015 sehr ähnlich sind, wurde für den ESA 2015 nicht erneut ein Antrag bei der Ethikkommission gestellt. Mit der Teilnahme an der Studie sind keine Risiken für die Personen verbunden. Eine absichtliche Täuschung oder falsche Darstellung der Untersuchungsziele findet nicht statt. Die zeitliche Dauer der Befragung ist somit als zumutbar zu beurteilen. Die Teilnahme an der Studie ist freiwillig. Die Einwilligung zur Teilnahme erfolgt nach dem Prinzip des „informed consent“. Durch eine Nichtteilnahme entstehen keiner Person, die zur Teilnahme am ESA 2018 vorgesehen war, Nachteile.
