Bij jongeren met een wegraking is de waarschijnlijkheidsdiagnose meestal een vasovagale reactie. De diagnose wordt anamnestisch gesteld; het kan daarna zinvol zijn verder onderzoek te doen naar uitlokkende factoren. Differentieeldiagnostisch dient vooral aan epilepsie te worden gedacht.

Indien bij ouderen in de anamnese geen duidelijke oorzaak wordt gevonden, is het zinvol te zoeken naar een cardiale oorzaak door bloeddrukmeting in liggende en staande positie, hartritmemeting en een ECG. Een verwijzing naar de cardioloog is alleen zinvol bij aanwijzingen voor een cardiale oorzaak.

Aanvullend onderzoek is alleen op indicatie zinvol; het levert zelden een onverwachte diagnose op en veel vals-positieve uitslagen.
