Summary
This article reviews early intervention programs for preterm infants and their parents. The Mother-Infant Transaction Program (mitp) involves 7 hospital sessions, just before discharge and 4 home sessions in which a nurse helps the mother adapt to their preterm babies. The Newborn Individualized Developmental Care and Assessment Program (nidcap) provides a clinical framework to attenuate the potential harmful effects of the early nicu environment on the preterm infant. It is based on the Synactive Model of Neonatal Behavioral Organization. Ongoing assessment of the behavioral cues of the infant and specific caregiving techniques decreases the stress response to the environment and promotes the physiologic stability of the infant on the nicu. Studies suggest that developmental care improves medical, neurodevelopmental and emotional outcomes, and reduces hospital stay. However, whether these results are reproducible in the Netherlands, given a different nursing education and health care system than in the United States, needs to be investigated. As a first step to introduce developmental care in the Dutch setting an early intervention program for parents of preterm infants, with use of the mitp and nidcap was developed. Parents need ongoing support long after discharge and the investements in the neonatal intensive care will only be fruitful when followed by long term follow-up and ongoing developmental care.
