Traitement endoscopique de la lithiase de la voie biliaire principale: résultats et indications

Conclusion
La place du traitement endoscopique de la lithiase de la voie biliaire est conditionnée par la présence de la vésicule biliaire, de son infection, du degré d'opérabilité du patient et de la certitude sur la présense d’un calcul cholédocien. Chez les sujets fragiles, la SE semble indiquée avant une éventuelle cholécystectomie. Lorsque le risque opératiore est faible, la coeliochirurgie est indiquée en première intention, notamment en cas de cholécystite associée. Si l’extraction par abord trans-cystique ou par cholédocotomie ne peut être réalisé par chirurgie laparoscopique, une conversion doit être effectuée avant de poser l’indication d’une SE, car en cas d’échec de la méthode endoscopique, une troisième intervention serait necessaire soit par chirurgie, soit par abord transcutané.
