Vaccinia gangrenosa

Summary

A case of vaccinia gangrenosa in a 3-year-old girl is described. The lesions were typical of progressive vaccinal gangrene, with rapid spread, tissue destruction and disseminated metastatic lesions.

Plasma studies revealed a normal level of serumy globulin. There was secondary infection of the lesion with mixed organisms. It appears that this patient was lacking in specific antibodies against the vaccinia virus, as after therapy with hyperimmune vaccinaly globulin, there was a dramatic regression of the lesions, with rapid healing. Secondary infection was controlled with antibiotics. Skin loss was made good with split skin grafts. Shoulder motion improved with physiotherapy. Patients with dermal complications following primary vaccination, must be treated early, with hyperimmune vaccinaly globulin. This will correct either a lack of specific antibodies or a delay in their production.
