Nigrotomy in treatment of extrapyramidal diseases

Summary

Thirteen patients underwent destruction of the nigral region for the treatment of extrapyramidal disorders.

The infratentorial lateral way of approach was used for these direct mechanical lesions.

The immediate and late results seem to be at least as good as those of pallidotomy or thalamotomy.

For evaluation of the operations in the area of substantia nigra it would be necessary to acquire further experiences.

Zusammenfassung

Bei 13 Kranken mit extrapyramidalen Störungen wurde eine Destruktion der Substantia Nigra vorgenommen. Für diese direkte mechanische Zerstörung wurde der infratentorielle laterale Zugangsweg gewählt.

Die Früh- und Spätergebnisse scheinen denen bei der Pallidotomie und Thalamotomie zum mindesten ebenbürtig.

Eine abschließende Bewertung der Eingriffe im Gebiet der Substantia nigra erfordert noch weitere Erfahrungen.

Résumé

Treize malades ont été soumis à la destruction de la région nigrale pour le traitement des dyskinésies extrapyramidales. Pour cette intervention mécanique directe on s'est servi de la voie d'accès latérale infratentorielle.

Il semble que les résultats immédiats et ultérieurs sont analogues à ceux de la pallidotomie ou thalamotomie. Pour apprécier les résultats des opérations dans la région du Substantia nigra il faudrait acquérir encore de nouvelles expériences.

Riassunto

In tredici pazienti portatori di disturbi del sistema extrapiramidale è stata praticata la distruzione della regione nigra.

La lesione meccanica diretta è stata eseguita attraverso una via laterale infratentoriale.

I risultati immediati ed a distanza sembrano essere almeno altrettanto buoni di quelli ottenuti con la pallidotomia e la talamotomia.

E' tuttavia necessario procurarsi una maggior esperienza prima di poter valutare completamente l'interesse di questa operazione.

Trece enfermos fueron sometidos a la destrucción de la sustancia nigra para el tratamiento de disquinesias extrapiramidales. Para esta intervención mecánica directa nos hemos servido de la via de acceso lateral infratentorial.

Parece ser que los resultados inmediatos y ulteriores son análogos a aquellos de la palidotomía ó talamotomía. Para poder apreciar los resultados de las operaciones en la región de la sustancia nigra hace falta todavía adquirir nuevas experiencias.
