New aspects of craniofacial surgery

Progress Symposium—Progress in Plastic and Reconstructive Surgery during the Last Decade

Abstract

The concept of craniofacial surgery, introduced by a French plastic surgeon named Paul Tessier, has completely transformed the treatment of facial deformities affecting the skeleton. Two of the more spectacular operations are correction of teleorbitism by orbital mobilization, and correction of facial retrusion by facial or fronto-facial advancement. These operations have evolved with the bipartition principle and the monobloc and splitting techniques. Craniofacial techniques applied in infancy for correction of craniosynostosis have proved very successful in most cases. The increasing use of miniplate fixation and utilization of cranial bone grafts are mentioned, as well as advances made in genetic and fetal diagnosis.

Résumé

Le principe de la chirurgie crâniofaciale introduit par le chirurgien plastique franÇais Paul Tessier a complètement transformé le traitement des malformations des os de la face. Les 2 opérations les plus spectaculaires sont la correction du téléorbitisme par la mobilisation de l'orbite et celle de la rétrusion faciale par l'avancement facial ou frontofacial. Ces opérations ont évolué grâce aux techniques de bipartition et d'avancement monobloc; les techniques crâniofaciales en particulier appliquées pendant la première enfance avec la correction de crâniosynostose réussissent dans la plupart des cas. L'utilisation croissante de fixation par miniplaques et des greffes d'os crânien, ainsi que les progrès effectués dans les diagnostics en génétique et in utéro doivent Être mentionnés.

Resumen

El principio de la cirugía cráneofacial, introducido por el cirujano plástico francés, PaÚl Tessier, ha transformado totalmente el tratamiento de las deformaciones del esqueleto facial. Las 2 operaciones más espectaculares son todavía la corrección del teleorbitismo mediante la movilización orbitaria y de la retrusión facial mediante el avance facial o fronto-facial. Estas operaciones han evolucionado gracias al principio de bipartición y a los principios de monobloque y división, pero también de las técnicas craniofaciales aplicadas en la infancia para la corrección de los craneosinostosis, han probado ser muy exitosas en la mayoría de los casos. El uso creciente de la fijación con miniplacas y la utilización de injertos óseos de cráneo, deben ser mencionados, así como los avances logrados en el diagnóstico genético y fetal.
