Zusammenfassung
Es wird eine Methode beschrieben, mit der ohne Zugabe von aggregationsauslösenden Substanzen die Plättchenaggregation fortlaufend photometrisch registriert werden kann. Aus einem 10 min lang rotierenden Kölbchen wird Plasma kontinuierlich abgesaugt und in einer Durchflußküvette die Änderung der optischen Dichte entsprechend der Abnahme der Zahl der freien Plättchen kontinuierlich registriert. Zusätzlich zu dem Winkel α1, der weitgehend den Ergebnissen des Plättchenaggregationstestes (PAT) nachBreddin undBauke entspricht, werden Beginn (tr) und Geschwindigkeit der Aggregation (α2/α3) erfaßt.
Klinische Untersuchungen mit dem photometrischen PAT ergaben bei bis 40-jährigen Normalpersonen in 24% (n=78) eine gesteigerte Aggregation und bei den über 40jährigen Normalpersonen in 45,4% (n=33). In der gleichen Altersgruppe (>40 Jahre) fand sich eine gesteigerte Aggregation bei peripherer arterieller Verschlußkrankheit (AVK) in 45,2% (n=73), bei Diabetes mellitus, länger als 5 Jahre bestehend, in 54,0% (n=87) und bei Zustand nach Herzinfarkt in 57,6% (n=59).
Summary
A photometric method has been developed, which allows continuous measurement of platelet aggregation without addition of aggregation inducing substances. Plateletrich citrated plasma is drawn out of a siliconized rotating flask (37°C, 20 rpm/ 10 min.). When aggregation occurs the number of free platelets decreases and the resulting change in optical density is measured in a flowing through-cuvette. In addition to the angle α1, which corresponds most closely with the results of the conventional platelet aggregation test (PAT), the onset (tr) and the speed of aggregation (α2/α3) can be observed.
The clinical investigations with the photometric PAT in the age-group below 40 years showed the maximum aggregation in 24% (n=78) of normal subjects and In the age-group of over 40 years in 45.4% (n=33) of “healthy” subjects.
In the age-group of over 40 years maximum aggregation was found in 45.2% of patients with arteriosclerosis obliterans (n=73), in 54.0% of patients with diabetes mellitus of more than 5 year's duration (n=87) and in 57.6% of patients with recent myocardial infarction (n=59).
