Zusammenfassung
Die Fundoplicatio mit der standardisierten Vorderwandtechnik ist die klinisch and experimentell bestens belegte Antirefluxoperation mit exzellenten Sofort- and Spätergebnissen, wenn man sie in Indikation und Technik gut beherrscht. Ihre Assoziation mit einer Vagotomie, bei exzessivem peptischem Element diskutabel, bei Begleitulcus am AusfluBtrakt unbestritten, hat wichtige Konsequenzen in der Art der Durchführung. Die Gastropexie allein ist in jeder technischen Modifikation gegen den Reflux ungenügend; sie bleibt unsere Operation der Wahl beim paraoesophagealen Magenvolvulus.

Summary
When the indications for fundoplication are correctly applied and followed, the standard anterior wall method has been proved both clinically and experimentally to be the best antireflux operation; it offers excellent immediate and long-term results. It is debatable whether it schould be combined with a vagotomy when the peptic element is excessive, for while this approach is undisputed in the presence of an ulcerated draining path, it calls for important considerations in its manner of execution. Gastropexy alone is an inadequate solution in all its technical modifications: We reserve it as the operation of choice for cases of paraesophageal volvulus of the stomach.
