171 diabetici di tipo adulto e senile trattati primitivamente o secondariamente con insulina sono stati sottoposti ad un tentativo di terapia con la somministrazione supplementare di antidiabetici orali. Il nuovo potente antidiabetico, glibenclamide (HB 419 = Daonil), è stato impiegato su vasta scala al fine di attuare una monoterapia orale nelle indicazioni appropriate, anche in associazione con biguanidi. Nel 52 % dei pazienti studiati, con la terapia orale supplementare è stato possibile ridurre considerevolmente la dose di insulina, nel 24 % dei casi si potè abolire del tutto l'insulina. In un altro 24 % dei casi controllati non si è avuta alcuna riduzione del fabbisogno insulinico. La riduzione del peso corporeo si è potuta ottenere in maggior misura nel gruppo dei pazienti regolati definitivamente senza insulina. Sorprendentemente, il successo di questa terapia orale supplementare non è limitato ai pazienti con fabbisogno insulinico giornaliero relativamente scarso ed in cui il pre-trattamento insulinico è stato di breve durata.

171 diabétiques de type adulte et sénile traités primitivament ou secondariement avec insuline ont été soumis à l'administration supplementaire de antidiabétiques par voie orale. Le nouveau très fort antidiabétique, glibenclamide (HB 419 = Daonil), a été émployé largement pour actuer une monothérapie orale dans le cas indiqués, aussi en association avec les biguanides. Dans le 52 % des sujets étudiés avec la thérapie orale supplementaire, les AA. ont réduit beaucoup la dose d'insuline, dans le 24 % des cas totalement. Dans un autre 24 % des cas étudiés on n'a pas eu aucune réduction d'insuline. La réduction du poids corporel a été plus significative dans le groupe des sujets reglés complèment sans insuline. Le succès de cette thérapie orale supplementaire n'est pas limité aux sujets soumis à l'insulinothérapie rélativement faible et chez lesquels le pretraitement insulinique a été de courte durée.

171 diabéticos de tipo adulto y senil tratados primitivamente o secundariamente con insulina, han sido sometidos a un intento de terapia con el suministro de antidiabéticos orales. El nuevo y poderoso antidiabético «glibenclamida» (HB 419 = Daonil), ha sido empleado en vasta escala al objeto de actuar una monoterapia oral con las indicaciones apropiadas, y en asociación con biguanidas. En los 52 % de los enfermos estudiados con la terapia oral suplementaria ha sido posible reducir considerablemente la dosis de insulina; en el 24 % de los casos se pudo eliminar totalmente el empleo de la insulina; y en el 24 % no se manifestó disminución alguna de las necesidades de insulina. La reducción del peso corpóreo se pudo obtener en mayores proporciones en el grupo de pacientes tratados definitivamente sin insulina. Sorprendentemente, el éxito de esta terapia oral suplementaria no se limita a los enfermos que tienen una necesidad diaria de insulina relativamente escasa y en que el pretratamiento insulínico fue de breve duración.
Zusammenfassung
171 erwachsene und Alters-Diabetiker, welche primaer oder sekundaer mit Insulin behandelt worden waren, wurden einem Behandlungsversuch im Sinne einer zusaetzlichen Verabreichung von oralen Antidiabetika unterzogen. Das neue stark wirksame Antidiabetikum Glybenclamid (HB 419 = Daonil) wurde auf breiter Ebene zum Zwecke einer oralen Monotherapie bei den angebrachten Indikationen auch in Assoziation mit Biguaniden eingesetzt. Bei 52 % der untersuchten Patienten war es moeglich, mittels der oralen Zusatztherapie mit der Insulindosis betraechtlich zu sparen; bei 24 % der Faelle konnte man das Insulin vollkommen absetzen. Bei weiteren 24 % der kontrollierten Faelle konnte man keine Verminderung des Insulinbedarfs feststellen. Die Reduktion des Koerpergewichts konnte man in groesserem Ausmass bei der Patientengruppe durchsetzen, die schliesslich ohne Insulin reguliert wurde. Ueberraschenderweise begrenzt sich der Erfolg dieser oralen Zusatztherapie nicht auf die Patienten mit einem verhaeltnismaessig geringen Tagesbedarf an Insulin und bei welchen die Insulin-Vorbehandlung kurze Zeit dauerte.
Summary
171 adult- and aged-onset diabetic patients treated with insulin primarily or secondarily were subjected to an attempted therapy with the additional administration of oral antidiabetic drugs. The new potent antidiabetic drug, glybenclamide (HB 419 = Daonil), was employed on a vast scale in order to carry out an oral monotherapy in the proper indications also combined with biguanides. In the 52 % of patients in study with the additional oral therapy the insulin dose could be remarkably decreased, in the 24 % of cases insulin could be at all abolished. In another 24 % of controlled cases no decrease in insulin need was found. A body weight decrease could be obtained rather in the group of patients regulated ultimately without insulin. The success of this additional oral therapy surprisingly is not restricted to the patients with a relatively poor daily insulin need and where the insulin pre-treatment was only of a short duration.
