197. Nebenbefund: Carotisstenose — Indikation zur Operation?

Summary

Among 943 own patients with arterial reconstruction of the extracranial cerebral circulation only 80 were asymptomatic since 1973. Serious complications had been 1 postop. death of myocardial failure (21st day postop.) and 1 neurological deficit. During late follow-up 12 patients died mainly of myocardial infarction, restenosis twice caused neurological symptoms, twice no symptoms. Prophylactic carotid artery surgery should be restricted to carefully selected cases.

Zusammenfassung

Unter 943 Patienten mit rekonstruktiven Gefäßeingriffen der extrakraniellen Hirnstrombahn fanden sich seit 1973 nur 80 ohne neurologische Symptomatik. Ein Patient starb postop. an Herzversagen (21. Tag), einer entwickelte eine bleibende Lähmung. In der Nachbeobachtungszeit starben 12 hauptsächlich an Herzversagen, 2mal traten neurologische Symptome als Folge einer Restenose auf, 2mal fand sich eine asymptomatische Restenose. Carotiseingriffe bei asymptomatischen Patienten sollten daher in streng ausgewählten Fällen vorgenommen werden.
