147. Erfahrung mit Leber-Echinococcus-Cysten

Zusammenfassung

Aus 444 gesammelten Fällen (davon 110 eigene) wurden folgende Ergebnisse erzielt: Betroffen sind Frauen 65%, Männer 35%, Alter zwischen 1–65 Jahren, am meisten aber 30–45 Jahre.

Lokalisation: Leber 60%, Lunge 30% und übrige Organe 10%.

Diagnose: Anamnese (Hund u. a.), Eos. in 90 % der Fälle. Casoni in 75 % +, Weinberg in 20% + und Immuno-elektrophorese 90%! Rö: Abdomenleer (Cystenschatten, Calcifikation) Schichtaufnahme, Artriographie, Szintigraphie.

Komplikation: Absceß, Ruptur mit evt. anaphylaktischem Schock, Urtikaria und Aussatz, Rezidiv.

Behandlung: Beste Cystektomie (in 20 % möglich), segmentäre Leberresektion, Marzupialisation (Vorbehandlung mit Formollösung oder hypertonische Lösung und Marzupialisation + Netzplombe).

Experience with hyatid cysts

Summary

An analysis of 444 collected cases (including 110 of the author's own cases) of hyatid cyst revealed that 65 percent of the patients were female and 35 percent male and that their ages ranged from 1 to 65 years, most patients falling within the age group 30–65 years. The liver was affected in 60 percent of the cases, the lung in 30 percent, and other organs in 10 percent.

The diagnosis was made with the aid of the anamnesis, of Casoni's intradermal test (in over 75 % of cases), Weinberg' test (over 20 %), and immunoelectrophoresis (in 90 %). The roentgenographic examinations used included abdominal X-ray, body-section roentgenography, arteriography, and scintigraphy.

Complications recorded were abscess, rupture, in some cases with anaphylactic shock, urticaria and rash, and relapse.

The best treatment is cystectomy (which was possible in 20 % of these cases). Other forms of treatment applied were partial liver resection, marsupialization (pretreatment with formol solution or hypertonic solution), and marsupialization with reticular plug.
