Zusammenfassung
Zwischen 1952 und 1973 warden 26 Patienten wegen einer Pericarditis constrictiva operiert. Es handelte sich um 19 Manner und 7 Frauen mit einem mittleren Lebensalter von 40 Jahren. Als Ursachen der Perikarditis warden in je 12 % Tuberkulose und Rheumatismus nachgewiesen.
Röntgenuntersuchungen ergaben in 69 % Kalkablagerungen am Herzen und Herzbeutel. Das Herz war in 23 % vergrößert, in 4 % klein. Neun Patienten wiesen präoperativ eine absolute Arrhythmie auf. Bei 2 Patienten bestand eine Mitralstenose, die gleichzeitig operativ beseitigt wurde.
Fünf Patienten sind postoperativ verstorben. Todesursache war in allen Fällen eine zunehmende Herzinsuffizienz. Zwei Patienten sind 15 bzw. 6 Jahre nach der Operation infolge chronischer Herzinsuffizienz bzw. Leberzirrhose verstorben.
Bei den überlebenden Patienten wurde in 69 % ein gates, in 26 % ein befriedigendes und in 5 % ein schlechtes Ergebnis festgestellt.
Clinical aspects, therapy, and prognosis of constrictive pericarditis
Summary
Between 1952 and 1973 26 patients with constrictive pericarditis were treated operatively. There were 19 males and 7 females with a mean age of 40 years. In 12 % tuberculosis and rheumatism were found to be the etiologic factors.
X-rays showed calcium deposits in 69 %. The heart silhouette was enlarged in 23 % and small in only 4 %. 9 patients had atrial fibrillation preoperatively. In two cases mitral stenosis was existent and treated simultaneously.
5 patients died postoperatively. Death was caused in each instance by cardiac insufficiency. 2 patients died 15 and 6 years postoperatively from congestive heart failure or cirrhosis of the liver.
The surviving revealed in 69 % good to excellent, in 26 % fair, and in 5 % unsatisfactory results.
