Summary
The clinical picture of congenital spinal epidural cysts is presented on the basis of 2 cases observed personally and 75 cases from the literature. The disease may present at any age; the interval between the onset of symptoms and the conclusive diagnosis is often very long. The long cysts extending over several segments of the vertebral column are mostly localized in the thoracic region dorsally to the spinal cord. Free communication between the lumen of the cyst and the subarachnoid space is seen in only some cases. The disease is sometimes chronically progressive and sometimes relapsing. It is impossible to establish a definite relationship between these two types and the existence or absence of communication between the cyst and the subarachnoid space. Clinical symptoms consist of more or less severe paraparesis or radicular disturbances according to the localization of the cyst. The most important diagnostic acids are the plain x-ray film of the vertebral column and myelography. These can almost always prevent confusion with a diagnosis of disseminated sclerosis or a vascular malformation of the spinal cord. Surgical removal of the cyst usually leads to very good remission.
Zusammenfassung
An Hand zweier eigener Beobachtungen und einer Zusammenstellung von 75 aus der Literatur zusammengetragenen Fällen wird die Klinik der kongenitalen spinalen epiduralen Cysten dargelegt. Eine Erstmanifestation der Erkrankung ist in jedem Lebensalter möglich; die Latenz zwischen dem Auftreten der ersten Symptome und der endgültigen Sicherung der Diagnose ist oft sehr lang. Die länglich geformten, sich über mehrere Segmente der Wirbelsäule erstreckenden Cysten sind überwiegend in der Thorakalregion dorsal vom Rückenmark lokalisiert. Eine freie Kommunikation zwischen Cystenlumen und Subarachnoidalraum besteht nur bei einem Teil der Fälle. Die Krankheit zeigt z. T. einen chronisch-progredienten, z. T. einen rezidivierenden Verlauf. Eine eindeutige Abhängigkeit der beiden Verlaufsformen von der Existenz oder dem Fehlen einer Kommunikation der Cyste mit dem Subarachnoidalraum ist nicht festzustellen. Die klinische Symptomatik besteht je nach Lokalisation der Cyste in einem mehr oder weniger ausgeprägten Querschnittssyndrom oder radikulären Störungen. Den entscheidenden Beitrag zur Diagnose liefert die Röntgen-Nativ-Diagnostik der Wirbelsäule und die Myelographie. Eine — insbesondere bei remittierendem Verlauf — nicht seltene Verwechselung mit einer multiplen Sklerose oder mit einer Gefäßmißbildung des Rückenmarks kann dadurch fast immer vermieden werden. Die operative Entfernung der Cyste führt meist zu einer sehr guten Rückbildung der Störungen.
