Der angiographisch sichtbare Kollateralkreislauf bei doppelseitiger Carotis interna-Thrombose

Zusammenfassung

Es wurde der Fall eines 44jährigen Mannes mit doppelseitiger CI-Thrombose in ihren extracranialen Abschnitten der Carotiden beschrieben. Mittels angiographischer Untersuchung wurde ein Kollateralkreislauf über die A. ophthalmica nachgewiesen. Diese Extracranial-intracranial-Verbindung war ausreichend für die Füllung der intracerebralen Gefäße sowohl im Raume beider A. cerebri anterior als auch der A. cerebri media.

Im neurologischen Zustand wurde eine rechtsseitige Hemiparese mit Aphasie sowie eine Schwächung der linken oberen Extremität festgestellt. Während des Krankheitsverlaufs machte sich eine mäßige Besserung der rechtsseitigen Hemiparese bemerkbar.

Die Literatur über die doppelseitige CI-Thrombose wurde kurz angeführt und besprochen.

Summary

A 44 year-old man was found to have bilateral thrombosis of the neck portions of the internal carotid arteries. The carotid arteriogram demonstrated a collateral circulation via the ophthalmic artery. This extracranial-intracranial communication was adequate as to maintain the needed collateral blood supply to the affected hemispheres — there was bilateral good filling of the anterior and middle cerebral arteries and their branches.

Neurological examination revealed a right hemiparesis with aphasia and weakness of the left arm. Moderate recovery from the hemiparesis ensued.

The literature on bilateral occlusions of the carotid arteries was briefly reviewed and discussed.
