Zusammenfassung
Auch wenn die transurethrale Resektion der Prostata (TURP) den Goldstandard in der chirurgischen Behandlung der symptomatischen Prostatahyperplasie bei kleinen Prostatadrüsen darstellt, werden derzeit bei Prostatavolumina >70–80 cm3 aufgrund des höheren Risikoprofils invasivere oder kostspieligere Interventionen empfohlen. In dieser kombiniert retro- und prospektiven Studie evaluierten wir die perioperativen Komplikationen und postoperativen Langzeitergebnisse von Patienten mit einem Prostatavolumen <70 cm3 und >70 cm3 nach TURP unter der Verwendung einer suprapubischen Absaugvorrichtung. Hinsichtlich Komplikationen und funktionellen Ergebnissen fanden sich keine signifikanten Unterschiede zwischen den beiden Gruppen. Die Modalität der monopolaren TURP bewährte sich damit auch bei Prostatavolumina >70 cm3 als ein sicheres und effektives Verfahren.
Abstract
The transurethral resection of the prostate (TURP) is well accepted as first line treatment for surgical management of lower urinary tract symptoms secondary to bladder outlet obstruction caused by benign prostatic hyperplasia (BPH) in small glands. In glands larger than 70–80 cm3, however, more invasive or more expensive surgical methods are currently recommended. In this combined retro- and prospective study, we evaluated perioperative complications and long-term outcome in patients with prostates <70 cm3 and >70 cm3 treated with TURP and applying continuous irrigation combined with suprapubic suction. No significant differences between the two groups regarding complications and functional outcomes were found. In conclusion, we state that the aforementioned modality of standardized monopolar TURP is a safe and effective surgical approach for prostate glands >70 cm3.
