Dissektionen hirnversorgender Arterien

Zusammenfassung

Die Mechanismen, die zu einer spontanen Dissektion hirnversorgender Arterien führen, sind bislang unzureichend verstanden. Es handelt sich um eine multifaktorielle Erkrankung, die nach aktuellen Vorstellungen auf einer präexistenten Vaskulopathie, also einer strukturellen oder metabolisch-entzündlichen Erkrankung des Gefäßsystems beruht, und zu der weitere Faktoren hinzukommen müssen, um das akute Ereignis der Entstehung eines spontanen Hämatoms in der Gefäßwand herbeizuführen. Die systemische Thrombolysetherapie innerhalb von 4,5 Stunden ist auch bei Dissektionspatienten ohne zusätzliches Blutungsrisiko möglich.
