Abstract

Purpose of Review

Malunion remains a common complication in the treatment of distal radius fractures. The purpose of this review was to discuss the various approaches in planning and surgical management for extra- and intra-articular distal radius malunions.

Recent Findings

Several recent studies have reported good results with surgical correction of distal radius malunions utilizing a number of preoperative planning methods and surgical approaches. Three-dimensional models and custom cutting guides have recently become more popular, but their benefit in comparison to other methods remains unclear.

Summary

Regardless of preoperative planning method or surgical approach, good results can be achieved with correction of distal radius malunion with careful attention to patient selection, indications, and surgical technique.

J Hand Surg Am. 2018;43:1036.e1–8. This in vitro biomechanics study confirms what has been previously noted in the literature regarding the kinematic effects of a malunited distal radius fracture, which may result in pain, stiffness, and arthritis. Still, this has not been proven and treatment of patient symptoms should still be the focus of care.

This study comparing conventional two-dimensional preoperative planning with three-dimensional preoperative planning with patient specific guides showed a trend toward a clinically important differnece in patient outcomes, but further study is needed to make any conclusions, especially given new technologies and a trend toward this more expensive planning method.
