Hintergrund
In Deutschland gibt es etwa 180.000 ältere Menschen mit Diabetes mellitus Typ 1. Bei schätzungsweise der Hälfte davon handelt es sich um geriatrische Patienten.
Problematik geriatrischer Diabetespatienten
Die sog. geriatrischen Syndrome wie kognitive Leistungsminderung, Instabilität oder iatrogene Störungen wirken sich ausgesprochen negativ auf die Fähigkeiten zum Selbstmanagement in der Diabetestherapie aus. So ist es eine neue Herausforderung für die behandelnden Ärzte, im Verlauf einer langen Lebenszeit mit Typ-1-Diabetes den Zeitpunkt festzustellen, an dem eine sichere, selbstständige Diabetestherapie vielleicht nicht mehr erfolgen kann und diese entsprechend zu vereinfachen.
Hilfreiche Instrumente und Techniken
Instrumente wie Hirnleistungstests, der sog. Geldzähltest, aber auch andere Funktionsuntersuchungen zur geriatrischen Beurteilung können zur Einschätzung der Fähigkeit des Patienten zum Selbstmanagement seiner Erkrankung herangezogen werden. Auch telemedizinische Möglichkeiten sind bei älteren Patienten mit Typ-1-Diabetes und stark schwankenden Blutglukosewerten sehr hilfreich.
Abstract
Background
There are approximately 180,000 elderly people in Germany with type 1 diabetes mellitus. It is estimated that roughly one half of these people are geriatric patients.
Problem of geriatric diabetes patients
The so-called geriatric syndrome with a reduction in cognitive peformance, instability and iatrogenic disorders has an extremely negative effect on the ability for self-management of diabetes therapy. This results in a new challenge for the treating physician, to establish the correct point in time during the course of a long lifetime with type 1 diabetes when a safe and independent diabetes therapy can possibly no longer be carried out and to simplify this in an appropriate manner.
Useful instruments and techniques
Instruments, such as brain performance tests, the so-called money payment test as well as other functional investigations in the geriatric assessment are useful. Telemedical options are also useful in elderly patients with type 1 diabetes and highly variable blood glucose levels.
