Zusammenfassung
Durch eine kontinuierliche Optimierung der First-line-Therapie beim Ovarialkarzinom durch Veränderungen von Substanzkombinationen und Applikationschemata ist mit Einführung der Platin-/Paclitaxel-Kombination als Standardprimärtherapie des fortgeschrittenen Ovarialkarzinoms seit Mitte der 90er Jahre bei fortgeschrittenem Ovarialkarzinomen eine Ansprechrate zwischen 70–80%, eine klinische komplette Remissionsrate zwischen 40–50% und eine pathologische Komplettremissionsrate zwischen 20–30% erreicht worden. Mit einem 10-Jahres-Überleben von 20% wird das Ziel einer kurativen Behandlung bei den meisten Patientinnen nicht erreicht (EBM 1).
Die Rezidivrate auch nach pathologischer Komplettremission liegt zwischen 40–60%. Die große Mehrzahl der Patientinnen mit Ovarialkarzinom wird auch nach optimaler Primärtherapie eine medikamentöse Rezidivtherapie benötigen (EBM 1). Das Ziel dieser Rezidivbehandlung besteht im Augenblick in der Linderung krankheitsbedingter Symptome. Für die Zukunft gilt es Second-line-Therapien zu entwickeln, die außerdem einen positiven Einfluss auf die Dauer des Lebens dieser Patientinnen haben (EBM 1).
Abstract
Improvement of primary treatment for ovarian cancer was accompanied only by minor progress in second-line therapy of recurrences. Up to now there is no curative treatment available, and the objectives of second-line therapies are the control of symptoms, low toxicity, and improvement of the quality of life.
The decision on treatment for ovarian cancer patients who experience treatment failure after initial chemotherapy depends on the response to prior treatment. Patients with recurrences after initial response and relapse >6 months are likely to be sensitive to prior drugs (platinum sensitive) and should be retreated with the initial platinum drug, with acceptable response rates. Patients who develop progressive disease during initial chemotherapy or who relapse <6 months are likely resistant to the initial regimen (platinum resistant) and should therefore receive alternative agents, although no single drug can be considered as the drug of choice, and the decision depends on toxicity and quality-of-life considerations.
