Extraforaminal Approach of Biportal Endoscopic Spine Surgery

Abstract

The extraforaminal disc herniation is a complicated degenerative disease, but its detailed knowledge is crucial for the proper performance of diagnostic and therapeutic procedures in extraforaminal biportal endoscopic spine surgery. The biportal endoscopic spine surgery allows us to use traditional spine surgical instruments in endoscopic procedure as in open surgery. The use of retractors to protect the neural structure makes this procedure safer than the uniportal endoscopic spine surgery. The use of two separate entrances (viewing and working), and at some distance from the skeletal and neural structures, leads to closer access to the target lesion by free manipulation of the scope and instrument rather than by a fixed view connected to the Kambin’s triangle. The extraforaminal approach of BESS (biportal endoscopic spine surgery) was a feasible and advantageous endoscopic technique for the treatment of foraminal lesions, including stenosis and disc herniation. In this chapter, the details of extraforaminal approach of biportal endoscopic spine surgery are discussed.
