Abstract
The incidence of tuberculosis has declined in the developed world but it is still an important problem in the endemic area like India. It is important for surgeons to know spectrum of manifestations of abdominal tuberculosis. Patients with abdominal tuberculosis have wide range of clinical presentation due to complex pathological process. The diagnosis is usually based on radiological imaging along with endoscopy. In majority of patients with acute presentation, the diagnosis is made during laparotomy and confirmed with histopathological examination of resected specimen. Common acute presentations include intestinal obstruction, perforation, and rarely gastrointestinal bleed. The other clinical presentation may be in the form of recurrent intestinal obstruction, ileocecal mass with uncertain diagnosis, fistulizing tuberculosis, and incidentally detected peritoneal tuberculosis. Management includes judicious combination of antitubercular treatment and intervention (surgery and endoscopic). Common indication of surgery is intestinal perforation, obstruction, failed medical therapy, and diagnostic uncertainty. Abdominal tuberculosis is potentially a curative disease with significant morbidity. This chapter is aimed at concise discussion of common surgical indications and surgical management of tubercular pathology.
