Abstract

Hypospadias is the second most common congenital genital anomaly. The hypospadias penis is often anatomically similar to the normal penis, at least as far as the dorsal surface is concerned. However, the common anatomical defect of hypospadias is a urethral opening that lies on the ventral surface of the penis. But the spectrum of conditions associated with ventral urethral opening in hypospadias is full of variations, including a ventral curvature of the penis (chordee), a ventrally deficient prepuce, torsion penis, abnormal raphe, disorganized corpus spongiosum and penile fascia, and many other conditions. A detailed description of the anatomy of hypospadias is next to impossible as each patient with hypospadias differs anatomically. Here is a brief account of the anatomy of the normal penis and hypospadias with attention to the availability of tissues, tissue configurations and its vascular and neuroanatomy. Anatomical understanding of surgical planes is pivotal in the selection of the appropriate procedure for hypospadias repair. This chapter will help us understand the finer aspects of this tricky yet treatable anatomical disorder.
