The Surgical Approach to Two-Stage Hypospadias Repair

Abstract

Proximal hypospadias is a unique and challenging variant of hypospadias. It is associated with a higher surgical complication rate and anatomic considerations that warrant an adaptive surgical approach. Over time our preference has shifted from a single to two-stage repair to achieve a more reproducible outcome. Assessing and proceeding with a two-stage repair consists of the quality of the ventral penile shaft and periurethral skin tissues, the degree of penile curvature, and the appearance of the urethral plate. The first stage operation corrects curvature and places supple shaft skin on the ventrum of the penis to facilitate a urethroplasty at the second operation. We stress the need for long-term follow-up and objective outcome measures to improve and deliver better outcomes to this challenging group of patients.
