The Importance of Margins in Sarcoma Surgery

Abstract

Sarcomas are rare tumours for which recurrence and metastasis reflect the local and systemic aggressiveness of disease. Adequate oncologic surgical margins are critical for local control of disease, and more recent literature would suggest that poor local management may have a longer term impact on survival. Planning of surgical margins is an important part of the strategy to offer adequate resection while enabling limb sparing surgery where possible. Modern imaging modalities and advances in radiotherapy and chemotherapy now allow safer resections without compromising either the removal of tumour bearing tissue or the need for tissue preservation. It is critical that all tumours that are suspected of being a sarcoma be referred for specialist care at a tumour centre expert in the management of musculoskeletal tumours.
