Addendum: vasculaire insufficiëntie van iliacale arteriën bij jonge sporters

Inleiding

Als jongvolwassenen tijdens sportactiviteiten diffuse beenpijn krijgen, dan is het vaak moeilijk te achterhalen wat precies de oorzaak is.1 Dit geldt vooral voor topsporters die de grenzen van hun fysieke mogelijkheden opzoeken. Meestal wordt gedacht aan verzuring van spieren door overbelasting, aan uitstralende pijn vanuit de lumbale wervelkolom of gewoon een techniekfout. Aan vasculaire oorzaken denkt men vaak niet omdat jonge sporters bijna nooit vaatafwijkingen vertonen zoals atherosclerose. Toch is het juist bij wielrenners , schaatsers , langlaufers en skeeleraars belangrijk om ook vasculaire insufficiëntie in de differentiaaldiagnose op te nemen. Maar liefs een op de vijf topwielrenners wordt beperkt in de prestatie door beenpijn ten gevolge van vasculaire insufficiëntie in een of twee benen.1,2

    Dissectie: losscheuren van de vaatwand.

    Hemodynamica = leer van de beweging van het bloed door de vaatwand.

    Fibromusculaire dysplasie = een niet-atherosclerotische en niet-inflammatoire erfelijke aandoening waarbij bepaalde arteriën zich vernauwen.

    Veneuze patch: ingezet stukje ader.

    Echodoppleronderzoek: hierbij worden echografie en doppleronderzoek gecombineerd zodat bloedvaten in beeld kunnen worden gebracht terwijl tevens de snelheid van de bloedstroom wordt gemeten.

    MRA: magnetische resonantie-angiografie.

  7. 7.

    CT-angiografie: dit is een combinatie van computertomografie (CT) en angiografie. Na inspuiting van een contrastmiddel in de bloedvaten wordt een CT-scan gemaakt. Na bewerking van de CT-data kan een driedimensionaal beeld worden gecreëerd van de onderzochte bloedvaten.
