Zusammenfassung
Die Behandlung der gastroösophagealen Refluxerkrankung (GERD) kann konservativ oder chirurgisch erfolgen. Eine sorgfältige Selektion derjenigen Patienten, die von einer chirurgischen Therapie profitieren würden, ist von größter Bedeutung für den Erfolg der GERD-Behandlung. Bevor die Indikation für einen chirurgischen Eingriff oder eine andere invasive Behandlung gestellt wird, ist neben einer genauen Anamneseerhebung eine objektive Einschätzung der Beschwerden notwendig. Hierfür sollte in jedem Fall eine 24-h-pH-Impedanzmessung zur Bestätigung eines pathologischen Reflux sowie eine hochauflösende Ösophagusmanometrie zum Ausschluss einer Ösophagusmotilitätsstörung erfolgen. Die laparoskopische Hiatoplastik und Fundoplicatio sind wesentlicher Bestandteil der Antirefluxchirurgie und stellen eine hochwirksame Behandlung der GERD bei gut selektierten Patienten dar. Die Fundoplicatio ist außerdem mit einer hohen Patientenzufriedenheit und einer verbesserten Lebensqualität bei Patienten mit chronischer GERD verbunden.
