Zusammenfassung
Das Kapitel Modulation zirkadianer Rhythmik beschreibt eingangs die Bedeutung von Schlaf und zirkadianem Rhythmus bei affektiven Störungen und im Anschluss die aktuell bestehenden Therapieverfahren, die durch Modulation von Schlaf und zirkadianem Rhythmus einen antidepressiven oder antimanischen Effekt erzielen können. Diese Therapieverfahren heißen Chronotherapeutika. Zu nennen sind hier die Lichttherapie, Wachtherapie und Schlafphasenvorverlagerung bei depressiven Episoden sowie die Dunkeltherapie bei manischen Episoden. Für therapieresistente depressive Episoden konnte insbesondere bei Kombinationsverfahren aus mehreren Chronotherapeutika eine Wirksamkeit nachgewiesen werden. Da alle Chronotherapeutika nebenwirkungsarm und gut mit pharmako- und/oder psychotherapeutischen Verfahren kombinierbar sind, stellen sie einen vielversprechenden Behandlungsansatz auch für therapieresistente affektive Störungen dar.
