Zusammenfassung
Das Frailty-Syndrom steht synonym in der Altersmedizin für das Nachlassen der Kraft und Agilität. Die körperliche Aktivität wird dabei maßgeblich von den körperlichen Reserven für die akute funktionelle Leistungsfähigkeit, aber auch körperliche Regenerationsfähigkeit bestimmt.
Eine Komponente dieses Syndroms ist die Sarkopenie, also der krankhafte Muskelmassenverlust mit Inaktivität und körperlicher Schwäche.
Dieser Verlust hat weitreichende Folgen insbesondere auch auf das Skelett und die Knochenmasse sowie die Körperhaltung. Diese Defizite gibt es zunehmend in das operative Management von verletzten und multimorbiden Patienten sowohl vor Operationen wie auch onkologischen Therapien zu berücksichtigen. Entscheiden sie doch maßgeblich über das clinical outcome. Moderne Therapie Konzepte planen daher die Erfassung der Sarkopenie und das Ausmaß des Frailty-Syndroms präoperativ in Co-Management mit der Geriatrie durch Assessments zu erfassen und gegebenenfalls auch präoperativ zunächst eine Stabilisierung in frührehabilitativen Einrichtungen wie der Geriatrie zu erreichen. Eine wachsende Zahl an klinischen Studien zeigt die Vorteile dieses Co-Managements nicht nur in der Alterstraumatologie und Onkologie auf.
Hauptbestandteile der Therapie sind eine Behebung der oft bestehenden Mangelernährung und Rekonstitution der Proteinreserven im Körper. Da die Ursachen der Mangelernährung komplex und multifaktoriell sind, sollte die Expertise eines Ernährungsteams bei der Versorgung des Frailty-Syndroms und der Sarkopenie vorhanden sein.
