Treatment of Refractory Myofascial Pain of the Neck and Shoulder Girdle with Botulinum Toxin A

Abstract

Therapy-refractory myofascial pain in the neck and shoulder girdle represents a not inconsiderable proportion of orthopaedic, accident surgery and neurological patients. In addition to oral/parenteral drug therapy, physical therapy, physiotherapy, manual medicine as well as needle procedures (acupuncture, dry needling) and shock wave therapy, infiltrative procedures play a major role in the treatment options, if necessary also supplemented by psychopharmacological and psychotherapeutic treatments. A number of substances are used in infiltrative procedures: Homeopathics, NaCl, various local anesthetics (sometimes in combination with corticosteroids), botulinum toxin. In this article, in addition to a general introduction to the modes of action and indications for botulinum toxin, this treatment will be presented and discussed on the basis of the literature, combined with a practice-oriented flow chart and classification in an overall therapy concept.
