Schlaganfall-Prophylaxe bei Herzinsuffizienz und Dissektion

Zusammenfassung

Es gibt klinische Konstellationen, in denen die optimale medikamentöse Prophylaxe bzgl. ischämischen Schlaganfällen noch unklar ist. Hierzu zählen die zervikale Dissektion sowie die Herzinsuffizienz. In diesem Kapitel werden die CADISS- und die TREAT-CAD-Studie vorgestellt, in denen untersucht wurde, ob bei Schlaganfallpatienten mit einer Dissektion der extrakraniellen Arteria carotis interna oder der Arteria vertebralis eine Thrombozytenfunktionshemmung oder eine Antikoagulation zu einer geringeren Rate an Schlaganfallrezidiven führt. Und in Hinblick auf eine medikamentöse primäre Schlaganfallprophylaxe bei Herzinsuffizienz wird die WARCEF-Studie zusammengefasst, die ebenfalls Thrombozytenfunktionshemmung versus Antikoagulation untersuchte.
