Zusammenfassung
Einnässen in der Nacht und am Tag gehören zu den häufigen Störungen des Kindesalters. Etwa 10% der Siebenjährigen nässen im Schlaf (Enuresis) und bis zu 6% im Wachzustand (am Tage) ein. Bei fast allen Kindern findet sich eine nicht-organische, funktionelle Ausscheidungsstörung mit guter Langzeitprognose bei adäquater Behandlung.
Die intermittierende Inkontinenz tagsüber wird durch verschiedene Formen einer Funktionsstörung des unteren Harntrakts verursacht. Hierzu gehören (neben einigen seltenen Formen) die überaktive Blase, der Miktionsaufschub und die Sphinkter-Detrusor-Dyskoordination. Die Enuresis wird eingeteilt in eine monosymptomatische (MEN) und eine nichtmonosymptomatische Form (NMEN).
Die nichtinvasive Basisdiagnostik ermöglicht in den meisten Fällen eine spezifische Diagnose der jeweiligen Ausscheidungsstörung. Mit der Standardurotherapie, gelegentlich unterstützt durch Arzneimittel oder durch Verwendung eines Weckapparates bei MEN kann den meisten Kindern geholfen werden. Wesentliche Komorbiditäten sind bei der Diagnostik zu erfassen und bei der Behandlung der Ausscheidungsstörung zu berücksichtigen.
