Zusammenfassung
Bei Patienten mit medikamentös schwer behandelbarer fokaler Epilepsie sollte die Möglichkeit nicht-medikamentöser Behandlungsoptionen, allen voran die Verfahren der Epilepsiechirurgie, geprüft werden. Die EEG-Diagnostik gehört neben den bildgebenden Untersuchungen und der neuropsychologischen Diagnostik zu den elementaren Bausteinen der prächirurgischen Epilepsiediagnostik. EEG-Untersuchungen im Rahmen der prächirurgischen Diagnostik müssen spezielle Voraussetzungen erfüllen, unter anderem wegen der geforderten Anfallsaufzeichnung, Länge der Registrierung, sowie potentieller Artefaktüberlagerung. Im vorliegenden Kapitel werden Grundprinzipien und spezielle Aspekte der Video-EEG-Langzeitableitung, EEG-Befunde bei verschiedenen Epilepsieformen, die Wertigkeit interiktualer und iktualer EEG-Befunde, sowie weitere spezielle Anwendungsgebiete von EEG-Untersuchungen im Rahmen der prächirurgischen Diagnostik (u. a. Wada-Test, Elektrokortikographie, usw.) dargestellt.
