Zusammenfassung
Am diabetischen Fuß sind Infektionen die häufigsten Indikationen zu Amputationen. Jeder Amputation muss eine exakte Befundung vorausgehen, damit die Unvermeidbarkeit belegt wird und die Amputation so weit distal wie möglich vorgenommen werden kann. Die bislang übliche Unterscheidung zwischen Minor- und Majoramputationen ist unzureichend, weshalb wir eine Klassifikation vorschlagen, die die exakte Höhe wiedergibt. Damit die Einschränkung nach einer Amputation in vertretbarem Bereich bleibt, kommen bis zum Unterschenkelniveau zehn Ebenen mit spezifischen Operationstechniken in Betracht, die wir mit Vor- und Nachteilen umfassend darstellen. Die Wichtigkeit der fachgerechten Nachbehandlung und der richtigen orthopädietechnischen Versorgung wird häufig unterschätzt, dabei sind sie wesentlich für ein gutes funktionelles Ergebnis.
