Zusammenfassung
Die infektiöse Endokarditis (IE), eine Infektion von Herzklappen bzw. intrakardialer Implantate, ist eine seltene Erkrankung mit hoher Mortalität. Prädisponierend sind kongenitale Vitien, degenerative Klappenveränderungen und kardiale Implantate. Die Diagnose ist aufgrund sehr unterschiedlicher klinischer Präsentationen oft schwierig. Die Bildgebung v. a. mittels Echokardiografie und Blutkulturen haben deshalb einen zentralen Stellenwert in der Diagnostik. Die häufigsten Erreger der IE sind Staphylokokken, Streptokokken und Enterokokken. Die Therapie erfolgt antibiotisch im Rahmen einer Langzeittherapie, in ca. der Hälfte der Fälle ist eine operative Sanierung nötig. Eine Operationsindikation besteht bei Entwicklung einer Herzinsuffizienz, mangelnder Kontrolle der Infektion durch Antibiotika und zur Verhinderung von Embolien.
