Zusammenfassung

Zum perioperativen Management liegen Leitlinien der HerniaSurge Gruppe vor, deren Aussagen zusammengefasst werden. Ob eine Antibiotikaprophylaxe das Wundinfektionsrisiko bei offener Leistenhernienversorgung reduziert, ist ungewiss, lediglich in einer Umgebung mit hohem Infektionsrisiko vermag eine Antibiotikaprophylaxe möglicherweise das Infektionsrisiko senken. In realiter ist die perioperative Antibiotikaprophylaxe bei offener Leistenhernienchirurgie aber weit verbreitet. In der offenen Leistenhernienchirurgie sollten wegen des Risikos des Harnverhalts Regionalanästhesie wenn möglich vermieden werden. Prä- oder perioperative lokalanästhetische Maßnahmen wie Feld-Block der Inguinalnerven und/oder subkutane/subfasziale Infiltration werden bei allen offenen Hernienversorgungen empfohlen. Da eine klinisch bedeutsame Serombildung nach Leistenhernienversorgung selten ist, wird eine Routinedrainage nach primär unkomplizierter Versorgung nicht empfohlen.
