Zusammenfassung
Candida-Nachweis im Abdomen ist häufig. Andere Pilze spielen abdominal kaum eine Rolle. Pilze sind fakultativ pathogen. Der Pilznachweis allein ist nicht hinreichend für die Therapieindikation. Ob Therapiebedarf besteht, richtet sich nicht nach dem Situs oder dem mikrobiologischen Befund, sondern nach dem Zustand des Patienten. Patienten im septischen Schock, mit reduzierter Immunkompetenz oder geschwächte Patienten mit wiederholten Laparotomien sind die Risikogruppen für Pilzinfektionen. Liegt tatsächlich eine invasive Pilzinfektion vor, so ist das gravierend, die Letalität ist erheblich. Keine Therapie ist erforderlich bei ambulant erworbener Peritonitis und blandem Verlauf (z. B. Magenperforation), nach Ansicht des Autors auch nicht bei Patienten mit Anastomoseninsuffizienz ohne Sepsis.
