Zusammenfassung
Durch den Einsatz multimodaler Therapiekonzepte sind Rezidive bei Rektumkarzinomen seltener geworden. Gerade deshalb stellen sie eine große Herausforderung für die behandelnden Ärzte dar. Neben dem hohen Metastasierungsrisiko, welches einen kurativen Ansatz häufig zunichtemacht, stehen technisch-operative Aspekte und Anforderungen an den Chirurgen im Hauptfokus. Er muss sich in einem häufig vorbestrahlten und bereits voroperierten Gebiet zurechtfinden und ausgedehnte, multiviszerale Resektionen vornehmen, um eine vollständige Entfernung im Gesunden zu erreichen. Nur so lässt sich ein akzeptables onkologisches Resultat bei insgesamt niedriger Mortalität erzielen. Der Preis hierfür liegt in einer hohen Morbiditätsrate, was jedoch oft die einzige Chance zur Kuration bedeutet. Letztlich bedarf es eines gut abgestimmten, qualifizierten und erfahrenen Teams verschiedenster Fachdisziplinen, um die Herausforderungen der Operation sowie des prä-/peri- und postoperativen Managements zu meistern.
